Welcome back to all Staff and Families for 2020
I am hoping this will be a great year full of exciting activities and lots of opportunities for us to get together and share information and experiences. This will help the Breakaway Team to better understand the supports you want and how we can provide these in the best possible way. Over the past couple of months, Breakaway has undergone some staffing and operational changes and I would like to take this opportunity to inform you of these. Staff Changes Frank Estevao has resigned his role as Service Manager. Many of you will be aware that Frank has been on extended leave and he has decided not to return from this leave. . Denise Upshall has returned to Breakaway and will take on the Service Manager role. Denise will ensure Breakaway moves confidently into the 2020 year with a focus on meeting the support needs of our clients through the delivery of high quality services tailored to individual needs. Please contact Denise at Breakaway or on 0409 622 196. Kayla Eaton has also resigned from the NDIS Officer role. Angela Hughes, who has been working with Breakaway for the past few months has accepted this role on a full time basis. Angela will assist you with the development of your Breakaway Service Agreement and also your Breakaway Participant Planner and Plan Management invoicing. Joe Presedo has agreed to take on the role of Client Service Manager. In this role, Joe will focus on building the Breakaway network for community participation and making sure that clients and families receive the supports they need, when they need them. Joe will be assisted by Claire Pavailler (Service Leader). Claire joins the Breakaway Team later this month and will I am sure, make herself known to all of you. Together Joe and Claire will design a more responsive and flexible community participation program. Samantha Lovett will focus on the Quality and Safety requirements for the organization. Sam will ensure all feedback received is addressed in a timely manner and reported and recorded to meet the new Safety and Quality Commission requirements. Sam will also work with Breakaway staff to ensure clients have the documentation they need to support them in their life choices, particularly in the areas of behavior supports and medication management. Amoy Hines continues in her role with Breakaway focusing on our internal and external communications such as newsletters, Facebook and the Breakaway web site as well as managing our IT systems ensuring Breakaway has capacity and capability to establish and maintain appropriate records and reports. Our Finance Team of Rita Agenbag and Elena Shlykova continue to support you with any queries you may have regarding invoices and payments. Operational Change Over the next few weeks I will be inviting family members and friends to joins us at Breakaway for morning or afternoon tea to discuss any concerns or questions you may have regarding supports and services and to share information with you relating to the NDIS and Breakaway changes and updates. Where possible a guest speaker will be invited to inform us on specific topics of interest and relevance. These meetings will be held every two months and are very important opportunities for you to have your say, ask questions and contribute to Breakaways’ quality improvement. Similarly, as Joe and Claire settle into their new roles, they will be looking at introducing some small group activities utilizing the Breakaway site at Bell St as a base for these activities. They will also be introducing some small group social outings over weekends and evenings as well as school holiday activities for our school based clients. These new programs will complement the existing music and movement program which has recently been revitalized. Please let Joe know if you are interested or have any thoughts on activities to include. NDIS Reviews Many Breakaway clients are being contacted for annual NDIS reviews. Just a reminder that if you receive Support Coordination from Breakaway, Tammy Buxton is able to support you in preparing for this review and is able to attend this meeting to provide further supports on the day. Please contact Tammy to discuss. If you do not receive Support Coordination from Breakaway, our team can still support your review by providing a summary of the supports received and how these have contributed to the NDIS goals stated on individual plans.
